You can add html to anywhere in your forms, so if you added something like

<img src="http://yoursite.com/image.jpg">

that would show an image where that line of code is in your form. You would have to host the image on your site or an image hosting service, but Wufoo allows for basic html to be added to the forms.

First off, wufoo is great. Us non-techies have the abilities to create function forms with great detail on the fly.

I do however want to make it a bit more functional for our customers. I have a wine submission form that I want wineries to fill out, but they may be entering multiple wines from the same winery and I would really hate for them to enter all the contact information each time. Plus they have to enter the compliance states for each wine as well.

I know this subject has been asked before, but maybe there is a work around? Or maybe someone has some suggestions?

Form is located here. http://www.radcru.com/submission_form.htm

Thanks a lot for any response.
Jeff
